Nutrition for Stress Fractures: Key Tips for Athletes

Nutrition plays a vital role in supporting athletes with recovering from stress fractures. Consuming the right nutrients can promote bone healing and help to optimize recovery.

Let’s explore key nutrients athletes should include in their meals and snacks when recovering from a stress fracture.

Nutrition for Stress Fractures: Consume Adequate Calories

My first priority when working with an athlete who has a stress fracture is to ensure the athlete is consuming adequate calories.

When athletes expend more calories than they consume in the diet, inadequate energy is available to support the health needs of the body. Underfueling and resulting low energy availability can negatively impact bone health.

Impact of Underfueling on Bone Health

Low energy availability leads to changes in bone metabolism, including a reduction in bone formation and turn over. These changes place both male and female athletes at a higher risk of experiencing a fracture (12).

In addition, low energy availability can cause a disruption in normal hormone levels (estrogen, progesterone, testosterone) and lead to irregular periods in female athletes (12).

Female athletes who experience lack of a menstrual cycle (amenorrhea) are at an increased risk for bone injuries. Hormones, such as estrogen and progesterone, are crucial for bone formation. Imbalances in these hormones can lead to reduced bone mineral density and negatively impact bone health (2).

Stress Fracture Nutrition: Underfueling and resulting low energy availability can negatively impact bone health.

Nutrition for Stress Fractures: Boost Your Protein Intake

Consuming adequate protein is also important for supporting an athlete with recovering from a stress fracture. Approximately 30% of bone mass is made up of protein. Following injury, protein intake is necessary to support bone remodeling and healing (3).

When an athlete is injured, their need for protein increases. Daily protein intakes of 1.6 – 2.5 grams per kg of body weight are commonly recommended for injured athletes (4).

The additional protein supports healing of the injured tissue and can help protect against muscle loss during periods of immobilization (4).

Spread Protein Intake Throughout the Day

In addition to ensuring an adequate intake of protein, injured athletes should be mindful to spread their protein intake throughout the day. Consuming protein with meals and snacks spread across the day can help maximize the anabolic (muscle building) effects of protein (4).

Pro Tip: Injured athletes should make sure to consume adequate protein with breakfast as well as with their evening snack (4). These are two time periods in which protein consumption is commonly lacking in an athlete’s meal plan.

Nutrition for Stress Fractures: Calcium

When recovering from a stress fracture, athletes should ensure they are consuming adequate calcium in their diet. Calcium is an important mineral that provides the structural component for building strong bones (5).

The recommended dietary allowance (RDA) for calcium varies based on an athlete’s age (5).

  • Children and teens, 9-18 years: 1,300 mg/day
  • Adults, 19-50 years: 1,000 mg/day

However, it is commonly recommended for athletes at risk of low bone mineral density or with concerns of underfueling to consume 1,500 mg of calcium/day to optimize bone health (67). This can be accomplished through a combination of calcium from dietary sources as well as supplements as needed.

Calcium for Bone Health: When recovering from a stress fracture, athletes should ensure they are consuming adequate calcium.

Nutrition for Stress Fractures: Vitamin D

Along with calcium, Vitamin D is necessary for optimal bone health. Vitamin D helps to promote the absorption and utilization of calcium by the body (89).

When an athlete has a stress fracture, I recommend bloodwork be done to assess the athlete’s Vitamin D level. If an athlete is deficient in Vitamin D it is common to recommend a supplement.

Dietary strategies alone (particularly since Vitamin D is found in few food sources) are typically not adequate to correct a deficiency (10).

Benefits of Dairy Foods for Preventing Stress Fractures

Dairy foods are associated with numerous health benefits for athletes. In particular, dairy foods are recognized for their positive impact on bone health.

Dairy foods provide key nutrients that are necessary for optimal bone health. Milk, in particular, provides 13 essential nutrients, including protein, calcium, Vitamin D, Vitamin B12, phosphorus, and zinc. All of these nutrients play important roles in the development and maintenance of strong bones (3).

A research study conducted on female cross-country runners examined the impact of dairy consumption on bone mineral density and risk of developing a stress fracture.

The study found that higher intakes of dairy products were associated with greater gains in bone mineral density as well as a reduced risk of stress fracture. The researchers noted that each additional cup of skim milk the athletes consumed per day was associated with a 62% reduction in the occurrence of stress fractures (11).

This is one more great reason for athletes to add milk to their recovery nutrition meal plan!

Ideas for Adding Dairy to an Athlete’s Meal Plan

I commonly recommend that athletes healing from stress fractures aim to include at least 3 servings of dairy in their meals and snacks throughout the day.

Simple ideas for adding dairy to an athlete’s meal plan include:

Pro Tip: Ultra-filtered high-protein milk can be a great addition to an athlete’s recovery meal plan. It provides 13 grams of complete protein per cup (12). Thus, it is a simple way to boost an athlete’s intake of protein while recovering from a stress fracture.

Milk for Bone Health: Milk provides key nutrients that are necessary for optimal bone health, including protein, calcium, & Vitamin D.

Importance of a Well-Balanced Diet for Stress Fracture Recovery

In addition to calcium and Vitamin D, there are numerous other vitamins and minerals that play a role in the development of strong bones. A diet deficient in these nutrients could negatively impact bone health. Thes nutrients include (3):

  • Magnesium
  • Phosphorus
  • Zinc
  • Copper
  • Manganese
  • Vitamin K
  • Vitamin C
  • Vitamin B12

Many of these nutrients interact with one another to support optimal bone health, thus highlighting the importance of a well-balanced diet when recovering from a stress fracture.

Nutrition for Stress Fractures: Consult with a Sports Dietitian

When an athlete sustains a stress fracture, I highly encourage the athlete to consult with a sports dietitian.

The sports dietitian can work with the athlete on developing a meal plan that supports recovery from the stress fracture. In addition, the dietitian can provide specific recommendations on supplements that may further support bone healing.
